The Annual Health & Wellness Assessment

The annual Health & Wellness Assessment plays an important role in helping you take charge of your health. This comprehensive assessment enables you to understand your current state of health and identify areas you may want to work on to prevent serious conditions. During your annual assessment, you will work with your provider team to build a plan to live a healthier life.

Before your annual assessment, you will be scheduled for a blood test. You may receive the results right away or within a few days, depending on the necessary tests. If the clinic has your current email address and you have been into the clinic for services since January 1, 2017, you may also choose to log into the Twine Health application from your computer or download the Twine Health application from your app store onto your mobile device prior to your assessment appointment. Through completing the Motivation Statement and the Health Profile questions in the “ME” section of the app, your provider team will have a more complete picture of your current health patterns in the categories of Lifestyle Behaviors, Mental Well-being, and Preventative Care.

If you don’t have a computer or you would like assistance logging in for the first time, please arrive for your appointment early and our clinic team can assist you.
