Health & Wellness Center Team

Gemma Lim-Go, FNP: Gemma completed medical school in the Philippines and practiced for ten years as a general practitioner. After completing her bachelor’s degree in nursing, she moved to the US in 2003 and worked as a NICU nurse for Valley Health System. She then received her master’s degree as a family nurse practitioner in 2011 and began working as a nurse practitioner in 2014. She has worked in various models, including retail clinics, urgent care, primary care, and specialty clinics. She has been serving her current community since 2014. Outside of work, Gemma loves to travel. Gemma is looking forward to serving Activate by Everside patients and being a partner in their journey to better health.
Medina Culver, DO: Dr. Culver completed her bachelor’s degree in cell biology and neuroscience at Montana State University, then attended Touro University in Nevada. She completed her residency in family medicine in Montana, moved back to Nevada, and has worked in a small private practice for the last three years. Outside of work, Dr. Culver enjoys anything outdoors, including hikes and walks with her golden retrievers. She also likes traveling, concerts, and live sporting events. Dr. Culver hopes to empower her patients to achieve their own health goals through continued health and wellness and to partner alongside them for the future.

Kathleen Wagner, MD

More than anything, Dr. Wagner loves “being people’s doctor,” supporting their health goals and accompanying them at times of difficulty. She feels that it is a sacred duty that she is fortunate to perform. She earned her Masters in Physiology from Georgetown University and completed her Family Medicine Residency at Jefferson Medical College in Philadelphia. During a professional break to raise her children, she also earned the Integrative Medicine Fellowship at University of Arizona, in an effort to broaden her approach to a more wholistic and spiritual orientation to patient care. In her free time, she enjoys time with her family, travel, knitting, quilting, music, cooking — and she is always reading.

Heather Tamburrino, PA-C

Heather Tamburrino is a board-certified physician assistant. She holds a Master of Science in Biomedical Science (MSBS) with a focus on physician assistant studies. When she’s not serving patients, she enjoys cooking, traveling, reading, home remodeling projects, and time with family and friends.

Eriko Suzuki, FNP: Eriko Suzuki, NP-C, has experience treating patients of all ages, helping them prevent and manage chronic conditions like heart disease, diabetes, kidney disease, depression, Alzheimer’s and more. Eriko earned her bachelor’s and master’s degrees in nursing and her Family Nurse Practitioner Certificate from California State University Long Beach. After beginning her career as the sole provider in a remote rural health clinic in Mammoth, California, she moved to Las Vegas, Nevada, where she worked as a nurse practitioner, treating patients with a high risk for frequent hospitalizations due to end-stage chronic diseases. Most recently, Eriko was the nurse practitioner for a dermatology clinic, where she helped patients with acute and chronic skin diseases, including eczema, psoriasis, rosacea, autoimmune disorders, skin cancer and more. When she’s not treating Everside patients, Eriko enjoys skiing, rock climbing and hiking in the mountains. And when she’s at home, she relaxes by baking and reading in her backyard.

The Trustees of the Plumbers and Pipefitters Union Local 525 Health and Welfare Plan are excited to announce the opening of Collective Union Health. What is Collective Union Health? It’s two provider locations strategically placed in the Las Vegas valley where participants and their dependents ages 3 years old and up can receive quality, comprehensive primary medical care services at no cost to you. It’s YOUR family medical center, powered by Activate Healthcare. Its purpose is to better the lives of all participants in our health plan.

We encourage you to take advantage of the free quality healthcare that not only provides participants with individual cost savings but has the potential for significant cost savings to the Health & Welfare Plan.

Medication Dispensing

Specific generic drugs are available through Collective Union Health at no cost to you and your family with clinic use. Because we are not a pharmacy, all drugs must be prescribed by the Activate Healthcare provider. If your provider has been prescribing one of these drugs and you want to take advantage of the drugs available at the clinic, please schedule an appointment with the provider at the clinic. If appropriate, they will write you a new prescription and can provide the drug to you. Per Nevada law, drugs can only be dispensed when the prescribing provider is on site at the clinic.

Lab Draw Information

Collective Union Health can now draw your labs onsite at no-cost! We can even draw labs ordered by outside providers. Give us a call at 702-960-4465 to schedule your lab draw appointment.

If we must send you to an outside lab, you will be sent to LabCorp. If you choose not to go to LabCorp, you will be billed and responsible for the charges.

Saturday Dates

One of the clinics is open every Saturday from 8 am to 12 noon, on a rotating schedule. Please schedule your Saturday appointment by calling the office.

Services for Children

Collective Union Health provides primary care services to children (over age 3) and to adults. Children between the ages of 3 and 18 must be accompanied by a parent or guardian for all services.


Are your immunizations up to date? Does your child need vaccines for school? Collective Union Health now carries select vaccines for children (ages 3 and up) and adults. Please call today to schedule an appointment and discuss your specific needs.
